The fuck you on about? The vast majority of them aren't. Medieval fantasy is a staple of WRPGs, and older JRPGs (DQ) that got inspired by them. Try naming a WRPG that isn't medieval or sci fi fantasy, and I guarantee you there's double that number of JRPGs, which actually use modern settings.

In most cases JRPGs use the same kind of fantasy setting but with a "modern" backdrop. You still end up fighting fantasy monsters with swords and magic in most cases. Something like Jagged Alliance where squad tactics and modern small arms are used doesn't really exist.
