its okay without mods. The only mod you really need is project nevada.
The expanisons are great, they add tons of hours of fun and are all connected as in, you will read something about one character in one dlc and in the other you will actually see that character.

Literally just download the bugfixes from the Nexus. If you're cool you'll download "more perks" and "perk every level".
Don't listen to anyone telling you to install rebalancing or overhaul mods. the vanilla game is fine.

Vanilla plus a bugfix that doesn't add features or cut content is the best way to play vanilla
Vanilla has a lot of game breaking bugs, and crashes every five minutes. I used YUP which was pretty good at stopping a lot of bugs and didn't add any extra bullshit
Still had to quicksave before every loadscreen though
Honest Hearts - Its alright, medium size world space to explore with some cool stuffed peppered in. Story isn't that great but it does introduce a pretty cool character that is talked about in the main game.
Dead Money - Amazing, one of the best DLC's ever made in a game once you get a grasp of things.
Old World Blues - Also amazing, really cool world space to explore and the story is enjoyable from top bottom. Probably my favorite DLC of the game but my inner snob paints dead money as being better.
Lonesome road - the bear and the bull, the bear and the bull, the bear and the bull. Its garbage, play it once, you will never want to play it again.

Very little polish on a game that isn't very good at it's core. Decent if you enjoy talking simulators.
