Step one to being commercially successful is to remain somewhat realistic. If you were being really ambitious, you could maybe aim for 150k copies sold.

At any rate, I'd just focus on a fusion of a good game with good faps. Outside of designing the game somewhat around making it easy for the player to get off (with a flexible camera and a replay mode, and perhaps sustainable, repeatable events), the best approach would be to just do these two things really well.

Also, a big problem for a western game porn dev is distribution. We don't have the Akihabara shopping district or mildly popular online game porn stores. So, that would need to be fixed.
