One item in AB+ lets you shoot diagonally but otherwise to shoot diagonally you need Mom's knife or correct character movement and aiming.

the game is pure rng shit, got it on christmas sale and played for around 4h, already finished it twice. If it required any skill at all it would take me longer than that
besides the shooting mechanics are dogshit, the hitboxes make no sense and unless you get a couple of nice items on the first two floors it becomes a bullet sponge shitfest

Strafing while firing tears with low to medium shot speed will get the job done, anyone that ever made it to haunt should know the strat. There is also dunce and wizard pills if you are looking for true diagonals.

That's as far as you're allowed to go on your first playthrough though. I did the boss after it without dying too, played challenge run stuff to try to make the game fun, then I realized the game is fundamentally horrible and nothing can repair it. The success of your run is determined by rng because the actual mechanics are too easy to fuck up. The game is extremely ugly, the endings are all shallow bullshit, everything about the game is terrible.
