I try to play on the hardest difficulty when possible. I tried with X-Com: Enemy Within's "Impossible" mode, and failed miserably. Hardest game I've ever played.

I DARE you to try XCOM: EW on Impossible Ironman with Red Fog, Damage Roulette and Diminishing Returns. And this isn't even the game's final form.
I just don't have the patience for it. I can't get past 3 missions on Impossible without failing. The amount of luck and planning required to win a mission is ridiculous.
I don't know how people beat that fucking mode.
It entirely makes me a bitch but I've toned down from the Nightmare modes in gaming because college stress is literally killing me and I need vidya to be a bit more relaxing atm.
When a games higher difficulties are just enemies are tougher and the player is weaker ,I find it boring. If it means I can get better equipment or level faster or anything else to go with it,then it is fine.

Some time ago a friend and I were discussing the idea of building difficulty into tge story. Higher difficulties meant the enemies were better trained and equipped.

Seriously fuck this shit. I BARELY play games to completion these days. I dont care how good your game is. Im not finishing it on a braindead setting so that I can play on a decently designed difficulty just so that you can pad out your game with 3 playthroughs to get some shitty "true ending".
Any "true ending" that just involves beating the game 17 times is fucking garbage.

Really, a game need to handle difficulty well first, then it needs to have sufficiently deep mechanics to benefit from the added difficulty.
RE4 is a pretty strong example of simple changes making the difficulty better.
