I honestly think that the ESRB and PEGI have something to do with it. This ONE trophy was apparently enough to push the age rating of Smash 3DS up from E10+ to T, which is why it was removed. I've seen all kinds of questionable content get past ratings boards (like Senran Kagura getting a T) while "cartoon violence" is enough to push a Nintendo game up from an E to E10+.
Is it just because Nintendo games are more likely to get into the hands of a general audience like kids and parents so the chance of them getting offended by a piece of content is higher than a more obscure game like Senran Kagura?
Pretty much, even jap games are butchered due to their new ratings laws.
With Smash specifically, I think the Tharja trophy was removed in Japanese versions so as to keep content the same across all regions.
CERO is questionable though. Just look at Fire Emblem

Should Fates really be given a C? Are Awakening and a remake of Gaiden really at the same age level as Thracia 776? And these aren't ratings from 20 years ago either. When put on the Virtual Console, the games got rerated (CERO didn't exist until after The Binding Blade, so the first six games received age ratings retroactively).

Region-lock is a non-issue, if you could read Japanese you would have imported a JP console and if you didn't you were reliant on hacks and fan translations anyway. The Switch not having region lock doesn't do a single fucking thing for people who would be affected by NoA censorship.

By the time I wait to see what games come out on the Switch and see if they get censored or not, I'll be waiting for whatever console Nintendo is making next instead.
Reminds me of how CERO wouldn't allow Hitler to be in Persona 2 PSPso they had to change him to Fuhrer and give him Sunglasses when they rereleased the game.
