Both, its a patch, but the major of the new content is locked behind a DLC paywall while a small part of it you get for free.

Generally speaking, the game moves away from its Industrial and World War focus for more medieval setting. You dont play as a country you play as a character of a dynasty, you marry, have kids, strengthen your dynasty, assassinate rivaling characters and try to get titles of yourself. then you die and play as one of your sons.
Thankfully there's a setting that lets you disable magic shit.
It's every man for himself, rather than factions. Games last much longer than one session. You play as a character rather than a country, and when your character dies, you continue on as your heir. Your expand territory by having your heir have claims to other fiefs (through marriage etc.) Traditional fabrication of claims is much trickier.
The most daunting initial thing will probably be wrapping your head around the feudal hierarchy of land ownership.
Counts -> Dukes -> Kings -> Emperors.

East African is actually surprisingly easy so long as you rush for your holy sites and spawn your holy order.
Combined with prestige army spam and your holy order doomstack, you can do some serious smashing to your Muslim neighbors. This will in turn leave them vulnerable to outside influences and internal Strife's very quickly. It's what u do every time as a Shi'ite or some tribal Muslim heretic.
Still, there's quite a bit of room for demise, especially if youre seen as a juicy target by the ummyads.

CK2 was my first paradox game and it didn't take me that long to learn the basics. After the tutorial I understood the gist of things then after a few more hours I had a firm grasp of the mechanics. This was with all the DLC.

Conquest is literally the best thing to do if you want more power and more game points.
