At night, you have feelings. Dreams are basically sensory imagery being summoned through a mysterious connection to those feelings. Dreams come from your blood, in a sense, and so do your instinctive feeling-responses. The game links up as much of human-reality to blood as possible, really. What's a blood echo? What's bloodtinge? It's fun.

Everything in the game might be a dream, but that doesn't rule out force of reality. I feel like the "dream of the game" is just echoing the player character's body trying to assimilate the blood he's given in the first cutscene, and that blood also contains this insane dream of Yarnham, the hunters dream and the nightmares and etc. If the player beats everything in the dream, it's as if he's beating back his own ailments one step at a time as well.
