definitely these things. I've come across them twice only during my some 500+ hours in dark souls. The first time was so confusing that I spent hours trying to fight it over again by dying in the same area.

It's like their spawning conditions were just fucked and From couldn't be dicked to ease up the requirements so more people actually saw these things on a regular basis. They're basically walking bloodstains, which would be fucking creepy if you came up to a dangerous area and there were two or three loot crabs wandering around. Instead they're so uncommon that the first time you run into one you might be convinced that something didn't load in properly.
What about that tentacle thing in Dark Souls 1 that was guarding Power Within in blight town?

fun fact: Vagrants actually came back in DS3. But they were patched shortly before the last DLC, and their spawning conditions are somehow even more obtuse this time around so almost no one has seen one outside the games files.

The tentacle thing was both a legit obstacle and a tone setter just like all the scripted dragon nonsense, as you can and WILL spot it on your first panoramic view of blighttown.
Souls 2 had a giant queen ant thing in the Gutter that farted gas and did fuckall and defended a room full of fucking nothing.
For it and the enemies in the room to be so fucking out of place hints heavily at some kind of lore, but its lost amongst all the OTHER stupidly out of place enemies like grave wardens and turtle knights.

Those octopus things at the bottom of Duke's Archives are weird.
I saw these 4 times total in all my Dark Souls hours, and all at the exact same place.
If I died in the area right before Four Kings where you can summon that witch and came back at some point, I always saw one.
That's a good spot to lose tons of souls, which is the main criteria. 4K was a huge roadblock to many.
I saw them once in the little grassy area near the bonfire under the red dragon, and once in the kiln. Both times it was that white one.
these were really weird, they have 2 ends but only one head so when you ran to it's other side the head from the front side would turn inside out and end up on the other side and it also has a unique animation when it kills you
