Doesn't Ninja Gaiden 2 do something similar, even going so far as to put Ayane's pink bow or flowers on Ryu to show you're playing the easiest difficulty? Or was it Ninja Gaiden Black that did that? It's been so long since I've played either.

Fun fact: this interaction and some others like it were specifically added to the game so that reviewers wouldn't rush through the game on the easiest difficulty setting and then shittalk the AI.
I honestly think the only times I saved in Postal 2 were when you had to kick the ladder just right to get to the Krotchy/Coleman fight and during the Gimp part on a no kill run since the buggy ass doors there could kill the rednecks and count against you
The autosaves for new areas are honestly all you need

It's been a long time since I've played GalCiv 2 but I remember there being a similar reaction to really aggressive cultural influence bombs. I can't imagine that those are the only two in the game.
