Over 100 million Wiis were sold. Where did all those casuals go after it brought them to gaming? Wii U sold just barely over a tenth of that. Everybody likes to pretend that it was because of a confusing name as if Wii owners weren't already used to buying shitty peripherals. The real reason is that these people are casuals among casuals and just wanted to try the hot new motion control fad. Once they got some basic party games they never felt the need to upgrade or try anything else. Gaming didn't cater to them, if anything the amount of shovelware pumped out for them pissed off AAA developers who began to abandon Nintendo.

Basically all of those consoles were sold to different consumers, not resold to the same consumer. Meaning a larger base of customers to sell more games too.

Actually the Genesis started this trend. The NES and SNES were seen as consoles for kids, while the Genesis was targeting older kids and preteens with its edgier games (Alter Beast was a big launch title for the thing, that should say something). The PS1 continued this trend by targeting teenagers as a whole.

No it was not inevitable, but Wii and PS3 had free online.
Left 4 Dead was supposed to have free DLC but microsoft cockblocked Valve and they were forced to sell any updates. This is also why Team Fortress 2 on 360 and PS3 had no updates since they also added content with each update. PS3 didn't get it free due to parity clause.
Most of Rare was still at Microsoft 5 years ago. Most of the best talent is still there believe it or not. Games like Viva Pinata shows that Rare still had it, but Microsoft only wanted them to make the new Kinect Miis
