Around 4:20, we can see Skull Face standing next to some XOF troops, and some jeeps.
The camera then leaves him, the troops, the jeep.

Then, around 4:46, we see two jeeps leaving the location. The first jeep clearly contains Skull Face in the back of the jeep.

We then see the location where the jeeps used to be, very clearly empty of any living humans.
The next time we see this place, Skull Face is somehow magically there again.

This apparent Skull Face then goes on to be killed/spared by Venom later in the mission.
But Skull Face isn't actually there.

We're seeing through Venom's eyes, and Tretij is present.
This is a hallucination.

Skull Face got away in Mission 31, and never died within MGSV.

I once met an ugly girl who didn't know if she was anime or realistic. Her face was weird and didn't fit with the game's art style at all.

There was meant to be an underground lab/factory stretching from OKB Zero to the Power Plant where you'd be given a tour over Skull Face's speech but Konami pulled the plug before it was finished.

However, they'd already done the cutscenes and mission design for the Sahelanthropus fight and they'd recorded the speech so the car ride was just a way to get that exposition out and move Snake to the right location.
