Snake and Solidus share the exact same face model in MGS2. As to why Snake looks different than Big Boss in MGS4? The artists probably just felt like it would be cooler.
He was horrifically burned, but kept alive in a coma by the Patriots. MGSV doesn't change this.

He was kept alive as a biomort. Basically a piece of meat that keeps living. The confusing part comes in when the BB biomort was replaced with a Solidus biomort and parts from Solidus' and Liquid's bodies were used to reconstruct BB's body.
