For game design? That's hard to say. It's like asking who's the most influential person in music, or film, even though so many genres exist within each that built successively off of the efforts of others before them. It's even more diluted in the case of game development, because it's rarely a single artist working entirely alone to realise his own vision. They're always teams of people who all contribute their own ideas to a project, and a figurehead like Miyamoto or Inafune end up taking a lot of the credit for the efforts of everyone else.

In any case, I would say the only singularly strongly influential individual who really had a solid impact on an entire sphere of the game development scene would have to be Pixel for creating Cave Story entirely on his own and virtually kick starting the modern indie movement.
