Bottom of the chair gave way and the guy got the cylinder joining the bottom of the chair and the wheels jammed up his asshole, which is why it's missing from the picture.

Most office chairs have a metal plate on the underside of the chair that rests against the gas piston to prevent exactly this sort of thing.
