Officially, no one has ever recoverd Kel'Thuzad's phylactery because they can't find it. Until his phylactery is verifiably destroyed, he can always come back.

He got offed in a vanilla raid, actually. But since he's a Lich I guess they had a semi-decent reason to bring him back for Wrath, where he got raided again (he also didn't really do anything in the entire expansion). Also his new voiceactor was horrendous.

Atleast he didn't get offed in a fucking 5-man like Anub'Arak.

There was an old quest line where in original naxx you gave his phylactery to a sketchy guy around lights hope and it kinda hinted that the silver hand might have some cult of the damned in it.
