In Modern Warfare 2 in one of first missions you massacre civilians on a Russian airport to stir shit up and start World War 3 basically. This mission was completely removed in Straya version and in Germany you weren't allowed to shoot.

Genuine controversy is pretty rare these days, mostly because controversy is often manufactured for marketing purposes. Given the corporate environment which video games exist in, it's even more rare that the decisions of one great artist, writer, or composer will make it through the machine untainted.
We do have a lot of 'soft' controversy still, but those typically belong in the category of 'manufactured' controversy.
