BloodGDX is a pretty good port. If you still want to play it in DOSbox and bought it on Steam or GOG, replace the woefully outdated version of DOSbox included with the game with the latest SVN build to improve performance and reduce bugs dramatically.

I'm playing through this for the first time and I'm trying to figure out how best to control it. At the moment I'm using WASD and mouse aim (horizontal only). Should I bother enabling vertical aim? Should I go full 90s and just use my keyboard?
finished it for the 1st time myself in 2015, with full mouselook. The BUILD engine's 3D enough to warrant using it.
Blood has full controller support and with the deadzone lowered a bit though external software, works just fine with a 360 controller.
Unlike Doom this game was built with vertical aiming in mind. You can still play with horizontal camera movement only but you will be limited when it comes to aiming your dynamite and other similar things. Get bmouse if you're playing on dosbox.
The java version is pretty spot on accuracy wise and runs great

I've done everything from installing bmouse to trying to play without mouselook altogether but no matter what I do it plays like dogshit.
Mouse always has delay and movement with mouselook (even with bmouse) is extremely imprecise and sluggish.

Its limited to four buttons and double press buttons for 8 total, but that's more of a limitation at the time.

Enemy accuracy is even worse, you can mindlessly run and gun through situations that would kill you in seconds in the original game. GDX doesn't have this problem, if anything the enemies might be a little bit too accurate there.

The one included with the online store versions was outdated when they were released. Also Bmouse never works well for me, I just don't use vert aiming.

Bloodgdx is so accurate that you can insert a mod is ridiculously complex as deathwish which relied on extensive knowledge and wizardry of the blood code and it runs almost flawlessly.
