It could be that the fog is tied to the first star, but a programming error flipped a bit after getting the star rather than tying it to the star selection which makes it disappear after getting the star. Not saying that is for sure what's happening but it's a possibility.

At The End of the World, there's a part where you enter pillars of light and go back to all the previous worlds for one room and do a fighting gauntlet. It's possible that that's how the Heartless got into those worlds to begin with other than gummi ship.
