The problem with microtransactions (even when it's just cosmetics, but especially if it goes further than that) is that it erodes the trust given by the player to the developer to make the best game he could. You're suddenly thinking things like if that epic item you need a big grind for is locked behind this effort because it's that strong and to make it feel special or because the developers saw an opportunity to offer a shortcut for money. It puts a permanent question mark between the entire effort-reward system of the game. Was this a gameplay and balance or a financial decision? Am I being a sucker right now?
Thoughts like this aren't good, obviously.
