I'm not sure of any character-exclusives outside possibly in Part 2. Also, Alice was basically the characters going through the game as they did in the original MGQ. Ilias involved commenting about how things "should" be and noting the differences.

For someone's who's never played Paradox part 1, how big of a difference is there in this choice?

The major difference is how the story plays out. Alice is basically similar to the story of MGQ, with talk about running around the world and collecting the four spirits, etc. Ilias is much more self-aware, pointing out differences which might be amusing to someone who's played MGQ already. Other than that, you get Alice/Ilias (and not the other) but Part 1 is the same outside that. Ilias does make the game considerably easier, although Alice has a lot larger pool of race options available.

Once you complete the story portion of Part 1, you're given the chance to restart with everything you have (including all levels and monsters recruited) and start the game again, so you can play through the game with the other character that way.

For the majority of both parts it only leads to different dialogue, but in part 2 divergence begins. It first starts with two potential boss fights in Alice's route being talked out of, but after that certain characters will be route exclusive. Of course they carry over to NG+ so you can still get all of them in the end. There will be a lot more exclusives in part 3 due to having to side with one of two factions.

Alice literally has her own game from start to finish with a satisfying ending etc,
Meanwhile best girl provides a refreshing change of character and just wants to help her grandson thrive and become a powerful angel worthy of her legacy.

It also has a massive job system, along with a similar monster "race" system. It's basically a lot like Dragon Quest 3, although where your job (in part) determines what skills you have access to. Monster race and even individual character traits also give access to skills.
