No, it doesn't translate well to 2D because the entire strategy for almost all fights is strafing around enemies. In 2D, you turn that into simply moving further back or behind enemies. Top-down could give you the same degree of movement, but at that point it's just the same game with a shitty fixed camera.

How about also updating shields to have more mechanics than parry and block. Have perfect time blocks or something that reduce stamina usage. Make shields more enjoyable, because as they were in Dark Souls they were boring and unbreakable, but in Dark Souls 2 and 3 they are next to useless unless you have some of the highest end greatshields or a parry shield. And this is before we get into how dodge spamming is the better strat anyways.

Go the opposite way of what DS3 did and bring back meaningful energy management and some semblance of interconnected world. Those things are an important part of the soulsborne formula, and I don't know if they land themselves well to 2d.
The perfect souls game would be like a full game like pre Lord vessel DS with ds2 mechanics such as Powerstancing, I frames on back steps or meaningful stats to push diverse builds.

What's wrong with it the way it is? Best case scenario you add in better hit detection systems that are more closely representative to the animations on screen. Collision detection improvements. All of it boils down to engine integration on From's side. The Surge is pretty mediocre because everything is a fucking sponge that does more damage to you than you do to them but the bosses are well designed and challenging. Hit detection and collision are fine in The Surge. Nioh is a departure from the Souls formula, treading more into the looter genre which is an interesting twist, combat in it misses the impact that Souls had despite the shared mechanics, feels more arcadey which I find detrimental. Dragon's Dogma borrows a lot from the Soulslike archetype, but falls through in content which was largely and transparently affected by the development cycle. The combat weight deficit it had is shared with Nioh. Where DD succeeded is build variety and attacks.
Bring in Souls' combat with advanced hit and coll detection systems, add in DD's build variety and skills, throw in some of that Nioh RNG gear with limited impact, add a highly refined BB dungeon generator, have an open hub but mostly linear story progression and use the proc-gen dungeons as a character progression system by providing RNG weapons that get stronger the deeper you get into the dungeons. Add Diablo 2 type multiplayer and supplementary skills for team play.

This is what I can't stand about Hollow Knight, and it's definitely something to gitgud with but I have zero desire to do so. If the shit isn't in an attack phase why the fuck is it doing damage to me and not me to it given I'm leaping on it's fucking head in most cases.

You may dislike certain things about DaS3 but it was objectively the most polished game in the series and had a lot of memorable moments with cool, interesting stuff that made this series better. DaS2 on the other hand was so disappointing that people try to defend it to this day.

Funnily enough, at least in DaS1, "perfect" blocks did exist. If you just tapped block at the exact time as the attack then the blocking animation wouldn't play. The window is so short though that it isn't worth doing, it's literally easier to parry.
