Noitu Love 2 had an ok simple vidya story, nothing special. Pretty much everything else in that game was better than its story.

That's true but Iconoclasts seems to be a lot more bloated. Cave Story meanwhile has a simple plot. Cave Story also has godlike pacing which has a lot to do with why the story fits so well

For a 2D action game, I would figure that making the overall character visually striking/pleasing would be the goal, not just artwork that doesn't match what the character looks like in-game.
It's like the character that is drawn is simply in costume as the character in-game, without actually being the in-game character in question.

If I'm reading you right, you're asserting that because the face isn't important during gameplay it shouldn't be featured in the promo art either? That's stupid.
For one thing, nearly all art depicting people requires that the face be attractive to the eyes (in both the literal and rhetorical senses) because that's how humans are coded to look at real people. The art shouldn't suffer or sacrifice this purpose just because it happens to be related to other art with a different purpose (in this case, the game itself).
Secondly, character design != drawing style. Tons of people routinely draw popular characters in different art styles, but they're still instantly recognizable because their designs are that good. Saying a piece of promo art doesn't match the design because it exaggerates different features is flat wrong, not least of all because the art likely came first. If you were right, then nearly every low-res sprite game would have this problem, because (again) in-game and extraneous art have different fundamental purposes which any artist would understand immediately.
Lastly, even if the left image didn't match the in-game look, I fail to see how the right does any better.

Alright then, let me ask you this: if fanart of this character gets drawn down the road, are the eyes/lips so vital to her design that, if redrawn by another artist, she would be completely unrecognizable? Or is her overall design more important to show "this is the character from game X"?
Simply pointing out that "her face looks fucking weird, it would look better if the lips/eyes weren't so PRONOUNCED", is absolutely not the same as suggesting "her entire design looks like garbage".
Focusing energy on design that shouldn't be the focal point seems like a very, very strange design choice to me, but if that is how the designer wants her to look, then okay. But when lots of people come back with "she looks funny".....well, that's criticism that could have been avoided with a redesign, or fans of this design should should be in enough control of their emotions not to fire back with things similar to "you just have no taste" when it's constantly pointed out.
