I'm happy to accept the dumb anime logic for almost every other MGS game, but in this one, especially where Miller's character development is honestly one of the few great things about it, is now muddied by whether he knew or not

Kaz 100% knew but went along with it because that was Big Boss's plan, that's why he kept getting so pissed off at Venom not doing shit he thought Big Boss would do. By the end of the game he hated Big Boss for abandoning him.

Big Boss does the same thing with The Boss in Peace Walker though. When he realizes she thinks war is gay he throws a tantrum and gets a nuke.

He didnt know, proves that. The only explanation is he is doped up, and with all the whole "Facts do not exist, there are only interpretations" line immmmediately following that scene, you can speculate all you want. Imo the nigger just had his limbs blown off so he was doped up. Or maybe he didnt want to know, convinced himself. etc etc. Maybe he has a brain tumor.

He "didnt get the surgery yet" at the time, the doctor was explaining the procedure to him when quiet came in

because Big medic didnt have shrapnel on your foreahead only. He has sharpnel all over his body before most of it was removed. Big medic before surgery looked very different from post surgery. Not to mention the actual face change.
Big Boss and Big Medic spent 9 years without any acontact away from MSF remnants after GZ. A lot happened to Miller during that time.

It was supposed to be a really good AI that was completely similar to the Boss. Whether or not that's true, Big Boss went crazy starting at that point because he thought the Boss "threw down her weapons".

The character creation screen isn't in any way a part of the world presented in the game but is a gameplay element. Just like the slowing down of town isn't canon or a part of that world. Although there are some gameplay elements (for example hud) which is canon because characters talk to you about it during the codec conversations but this isn't one of them.
