Nothing, Nintendo even provides the stencils for you

It comes with a game to interact with the cardboard and reflective type that the IR sensor of the joycons detect. If they sold the game separately, I guess nothing's stopping anyone from making their own devices from scratch, and I wouldn't be surprised if Nintendo themselves encouraged anyone creative and able enough to come up with their own bootleg to do just that.

Because it comes with pre-cut cardbaord parts as well as the required accessories needed to make it work. It's $20 for the convenience of being able to build the devices right out the box.

The cardboard is laser-cut, pre-painted, and includes all the supplies you'll need to make it. So I guess it's like paying Nintendo an extra $20 to make it for you.
