Sucking Kojima's penis will sometimes be difficult because the antagonist, The Hollow, which is actually an imaginary representation's of Kojima's fragile ego swallowing everything, makes Kojima's penis go soft in fear. The twist at the end is that you are actually a fan that kidnapped Kojima and is forcing him to make a game in exchange for oral sex.

Had a dev budget of 80 million which is relatively modest. GTAV was 250 million for comparison

GTAV has one of the highest production budgets of all time by a significant margin. The usual AAA game development budget is around the $60-$80 million mark. That is your point of comparison. MGSV did not have a modest budget at all, it was around the high end of a AAA development budget.
