mad dummy and napstablooks themes are the same
Mad dummy's theme is a "remix" of napstablooks.
I'd argue that this counts and is appropriate.

Like 40-50% have their own themes, not all of them.
Not that guy, but every trial boss has their own theme and every raid tier finale has their own theme. Dungeons, however, do share a boss theme with the rest of the dungeons from that expac for the most part.
Also sometimes dungeon bosses introduced in new patches have different boss themes from the most recent expansion
your key problem is pretending dungeon "bosses" are difficult or relevant enough to even be considered bosses
That's what I meant by "for the most part." And also stuff like the Fractal Continuum (Hard) which brings the old boss theme back.
Actually, I didn't meant dungeon bosses. I thought about, for example, the Warring Triad sharing the same theme. Whole tiers of raids/savage sharing the same theme, by the exception of the last boss.
That's why I said 50%
Warring triad phase 1 is the same theme but phase 2 is unique.
I think the first phases of the Triad is the only time any trials share any music at all, actually. Though that first phase makes up tiny proportions of each fight.
Even though it's used for a bunch of bosses,
____Locus____ is definitely my favorite theme in the game.
Aside from a couple of reused tracks and themes in Dark Souls 1 and Bloodbourne most bosses music in the Souls series is unique to them.

Ar Tonelico has unique themes for most bosses, and it is some of the most spectacular you will ever hear.

Not him, but I'm guessing it's Deus Ex. Areas have five different tracks:
>exploring
>combat
>talking with an NPC
>a brief track that plays when you die
>a brief track plays when you exit an area, usually by helicopter or boat
These are unique for each area and some of them change with story progression (e.g. the music in unatco changes when you're escaping from it).
