Maybe your friend was one of those epic Counter-Strike hackers that download free wallhacks out of mpgh or God knows where. That shit tends to be infected with RATs.

Well, I haven't seen your friends files. But if he said he left some ports open and his router allowed outside access, then the hacker could've injected a rat on his system. So it's possible. Or he just downloaded some wallhacks that are infected and send a message to the creator of the hack so he could troll users.
Eh, we'd just blue screen the computers across the network we were playing on. Most of them ran win98 anyway.

He might have played on a custom server and the game downloads the mod files before you join a server, this has been the case since HL1.
So one of these mods might have had this functionality. That's it.
No hack, no port shit, just the game + mod.
