You start from scratch by creating a OpenGL-context and a window. I use GLFW and GLEW to get something up and running quick. Then you add features such as drawing a few polygons. Put textures on them. Add lights. Add the interaction by changing where to "look" by reading the mouse x & y position. Add shaders and more visual eyecandy.

Keep on adding features such as physics: can this object stand on this object? Expand it slowly with functions and features.

Building 3D engine might sound big and scary but it's really just time and good documentation that's the key parts of success.
