It's a map of small towns. Cairo is just one dot, but so is the small town next door. That's why in Germany it looks nothing like a map of the population density. Instead rural areas like Mecklenburg and Schleswig-Holstein in the north are dark.

The way the states organize their municipalities also plays a role. Some incorporate several settlements into larger villages and towns while other use a sub-division between the county and municipality level.
So it's more like a map of the administrative history and has no real value.
