I am looking for the name of a 80's arcade game. You start as a plane with a series of enemies flying pattern formation. There were 4 power ups and your plane took a different shape with every variation. Every time you were shot you lost power ups. If you kept all of them, you tuned into an invincible Phoenix for a short period.
