My understanding is that they had a small number of turnstiles and after going through them you went to the pen you wanted to (I.e. your pen was not on your ticket). The police were not monitoring the situation closely to guide people away from the pens with the crush and thus fans kept moving into them (fans at the back would have no way of knowing a crush was occurring toward the front). The report said that for many games you were assigned a pen but this was not the case for a sold out semi final

Even if there were a few that did this, not having proper security to prevent them from doing this is unacceptable.
