From what they tell us, she mostly just puts on a show in life. She uses Koyomi to give herself a purpose in life, so she is essentially using him for that. Even wants to go as far as to kill him so that she can always say she has a perfect love she can never have
Kaiki doesn't bring her back to reality, he just convinces her that there are better goals to go after than him and she kind of agrees.
I don't think she can really love or anything and she is one of the few characters they leave broken in the series.
