that's the 1976 release on Black Saint - credited to Don Cherry, Dewey Redman, Charlie Haden & Eddie Blackwell, album name "Old and New Dreams"
there's *another* release from 1979 on ECM - album name again "Old and New Dreams", but often credited to "Old and New Dreams" (although it again has the same individual names on the cover). Attachment related.
These are totally different albums with different tracks. Confusing, I know.
