Father's name plus a suffix indicating belonging, so it kinda resembles some western "name+son" surnames. At least that's how it works here.
Example: Vladimir Vladimirovich Putin. The Vladimirovich part means his fatner's name was also Vladimir.

In everyday life we use the "given name + family name" though.

However for common people it started in the military in the modern era, were there was a need for the administration to tell people apart so people took surnames. However it wasn't removed entirely until as late as 1966, at which time anybody still using a patronymic name got it as their surname.
