Rules for American tipping:
10% - terrible service, more of an insult than leaving nothing, done to show how displeased you were
20% - acceptable service, nothing deliberately done wrong but slow inattentive wait staff
30% - average service, you received the minimum level of service you could reasonably expect
40% - great service, the wait staff put extra effort in to improve your experience (e.g. making sure you are happy with your meal, refilling drinks within 10 minutes of them being empty)
50% - exceptional service, the wait staff have either gone balls out to meet your every need or you've caused them to go to extra trouble (e.g. dropping silverware, spilling drinks or moving you to a different table at your request)

Managers of restaurants are required to subsidize the tips/low wages of servers until it meets minimum wage. If the server isn't reaching minimum wage, it's something they need to take up with their employer.

In most states if not all, it is a legal requirement to make the employer to cover the difference between "tip wage" (something like 3 bucks/.hr) and the local minimum wage.
So if Saly makes 3 dollars and min wage is 7.50, the employet still HAS to pay her another 4.50. Now that's not true completely, if she made 1 dollar in tips, then the emplyer only has to pay another 3.50.

They actually do sometimes. I actually tried to tip 'american' once and gave a restaurant I frequent a 20% tip. Our server, the owner, said to me that she can't accept it as it is 'too much'.

No, working at fast food requires little to no service skills. while being a waiter or waitress involves more, as well as helping customers navigate a menu. Being a bartender is even more responsibility as you have to know how to mix drinks properly, multi task, give good service and give opinions and direction on menu items.
