Looks like cellulitis as a result of a bacterial infection.

Most likely staph. aureus or strep. pyogenes, both of which should be sensitive to the antibiotic you're on. Tell your doc treatment isn't working as the bacteria might be resistant to the antibiotic, and you might need to switch meds.

Infections like this can happen super randomly, and from the smallest of cuts. Even the cracks in the skin on your toes can create a passageway for these bacteria to enter.

I actually have a colleague out in California who specializes in this procedure. I however do not do it.

So lets say I inject someone with drug X. Serum concentration is 12.5mg/L. Two hours later its 10mg/L. Assuming first order kinetics, whats the concentration after two more hours?

I don't recall the equations to calculate 1st order kinetics (I don't think I've ever had to do this on an exam), but drugs that follow zero order kinetics are metabolized as a constant amount per unit time, whereas with first order kinetics the drug would be metabolized as a constant fraction per unit time.

Lots of drugs follow first order kinetics metabolism, which is why there is the need of reaching a steady state. However some drugs follow zero order, such as alcohol. (metabolize 1 drink an hour regardless of how much you've had)

It could be pancreatitis or hepatosplenomegaly (enlarged spleen and liver). You'll need an actual physical exam to determine if your spleen and liver are enlarged. If they aren't, it's most likely pancreatic.

You are also at risk of a folate deficiency as alcohol inhibits your ability to absorb this necessary vitamin. I'd recommend taking a supplement of folate and thiamine (another vitamin commonly deficient due to alcohol)

I like to think of first order elimination as a constant % being eliminated (for example referring back to your question) going from 12.5 mg/L to 10 mg/L, 20% was lost. which leave 10mg/L. Another half life will result in another 20% loss from 10mg/L (20% of 10mg is 2mg) thus 8mg is your answer

I met my mentor, a surgeon, during my junior year. I could tell that he was happy to leave his bed every morning and meet with his patients - you could see it in his stride as he moved about the clinic. Then, my second love was being in the OR with him. After that experience I knew that I had to do medicine.
