I don't have the screencap on me at the moment, but it basically went along the lines of this dwarf getting injured, I think fire.
Doctor dwarf tried to fix him up but ended up creating this monster of a dwarf that went full-ham on his fort and killed everyone, all while screaming in agony.
Makes a new fort elsewhere, this dwarf comes screaming over and does the same.
Dude couldn't play on his world any more because this dwarf ended up reappearing at some point and murdering everyone.
Can't remember version it happened in.
Also don't have the save any more because of a drive failure. RIP files ;_;
