Bruno Ganz based his voice off the only private recording there was of Hitler. The Finns secretly taped him on a train, he sounded different in his actual speech opposed to the bombastic speeches.
Hitler also looked old as shit in 1945, just look at some vids. The stress of leading a country at war would understandably age a person prematurely. Also he was pretty much a tweaker.
