With a cursory understanding of how the rankings work, it seems like it would be extremely challenging for a team not in UEFA/CONMEBOL to climb very high in the rankings. They don't play enough games against upper ranked teams to get enough points to rise, outside of WC matches. This is not to say that they necessarily deserve to be higher ranked (Australia shouldn't be near the top for stomping on other oceanic teams constantly), but how they give points seems to keep UEFA/CONMEBOL at the top
