In 2029, mutants are on the brink of extinction due to a virus created by a government program called "Transigen". Logan and Charles Xavier are living in the Mexican border, where Logan is working as a chaffeur and caring for an ailing Xavier, who is suffering from a degenerative brain disease which induces seizures that disrupt Xavier's powers. In one such incident, Xavier telepathically killed the X-Men and numerous innocents and had to flee the country.
Logan himself is slowly dying from Adamantium poisoning after the virus severely weakened his healing factor, and all he wants is buy a sailboat so Xavier and him can spend their remaining days away from civilization. Logan believes them to be the last mutants, but Xavier has been having visions of a young girl. Logan is helped by a tracker, Caliban.
One day, Logan is approached by a woman, Gabriella, who is being hunted by the government and wants Logan to escort her and her daughter Laura to the Canadian border. Logan refuses, and Gabriella ends up dead, so he takes Laura back to his hideout, but they are followed by Donald Pierce and the Reavers, cybernatically-enhanced mercenaries working for Transigen who want Laura back. Xavier realizes she's the girl he's been dreaming about and convinces Logan to protect her. Laura turns out to have similar powers to Logan and the trio goes on the run, while Caliban is captured by Pierce and forced to track them.
Logan and Xavier find out Laura is Logan's daughter, born from genetic material that Transigen retrieved from the Weapon X Program that was artificially inseminated on Gabriella; and that Gabriella and Laura were searching for Eden, a fabled safe haven for mutants that Logan doesn't believe exists. Xavier persuades him that they need to take Laura to the border.

After escaping from the Reavers in Las Vegas, Logan, Laura and Xavier take shelter with the Munsons, a kindly family that's being run out of their property by inescrupulous land developers. Meanwhile, Transigent director Zander Rice deploys X-24, a clone of Logan, to retrieve Laura.
Logan helps the Munsons fend off the land developers and they find a semblance of peace until X-24 attacks and kills the Munsons as well as mortally wounding Xavier, while Logan and Laura escape. After burying Xavier, they eventually reach Eden, where a group of mutant children born about the outbreak are being protected.
Rice, Pierce and their men attack Eden hellbent on capturing Laura and the young mutants. Caliban sacrifices himself to hold them off, and Logan is given an injection that boosts his healing factor enough for him to attack and slaughter all the Reavers and Pierce, while Laura kills Rice and retrieves an Adamantium bullet that Rice kept to dispose off of X-24 should he go rogue.
Logan battles X-24 and manages to injure him long enough for Laura to shoot him dead with the Adamantium bullet. Logan is impaled on a tree stump in the process and dies in Laura's arms. She buries him in a makeshift grave and, before leaving to rejoin the children of Eden, she tips the cross marking Logan's grave to form an "X".

As for the franchise, it'll keep going. Now that they've pretty much embraced that they're playing loose with actual continuity we're getting the mainline X-Men movies set in the 90's as well as the Deadpool movies set on present day of what is assumed to be a parallel universe and then there's the Gambit and New Mutants movies who are shaping up to be standalones as well.

LOGAN is ostensible in the same continuity as the "good end" from X-MEN: DAYS OF FUTURE PAST. However, there are no direct references to past movies, and at one point there's a vague implication that the events of the previous movies as we know it are a slightly romantized version of what truly happened much like the X-Men comic books that Laura carries around.
So most of the stuff from the previous movies technically still happened but not as we think.
