A lot.
Unilateral secession from Spain necessarily implies unilateral secession from UE. It does not matter that they want to reintegrate in the EU, the EU is a confederacy of states and it relies on the states to determine what countries actually are.
Nobody knows what the actual fuck will happen if they declare independence. Like, what is the legal status in the Schengen area of goods and people coming from Catalonia? Because the EU will not recognize the independence of Catalonia and will accept whatever Spain says, they will have to accept anything coming from Catalonia as something coming from within the EU. But that would be only in theory, in practice if Spain is not able to establish its sovereignty in that territory, and since the EU can not directly speak to the Catalonia government as it was a state member, it means that the EU legislation is not actually working in it. It is legal for France to re-establish border controls in the Catalonia border?
And then there are the economic consequences. Spain is a sizeable country in the EU so its government debt weights a lot; defaulting on that debt would pretty surely cause an euro breakup. Without catalonia, Spain can not pay the government debt back.
Catalonian independentists are not only trying to blackmail Spain, they are also blackmailing the rest of EU to attempt to force its acceptance in the club.
This is the first time in the history that something like this happens in the EU, and it would set a precedent. It is in the best interest of the EU that secessionist rednecks are punished as much as possible. So my guess is that they will let Spain to play hardball with them.

EU can't let them in, even the really pro-union types can't let them. Because 1:
1. Spain would resent the EU and entering the union takes an "aye" from every single EU member.
2. Lots of countries in the EU have separatist movements. If they think the EU will play ball with the people that would break up their countries, those countries' leaders would want out of the EU.

Neither Spain nor EU members or EU allies recognize Catalonian Independence. They may withold EU funds from the region, and effectively leaving Catalonian interests without representation, but they won't block movement of people and goods and Catalonians already feel they aren't being heard anyway.
