Not always, I had a minor case myself and they were able to preserve it. I did stretch it to make it longer via non-surgical foreskin restoration methods later to make it longer, though.
Despite hearing some horror stories, my end result was great even years later, it effectively looks like a normal dick without a frenulum (it's "flat" there).
It may have been born malformed, but I ended up crafting a pretty ok dick myself with the help of a skilled surgeon.
