I think the idea is that every pali and straight gets you a disease tile instead of a role. You can use said tiles on your enemies. It turns 8 of their tiles into diseased lands and expands one tile per turn. You have three. But its also why you don't have as much real territory. You have 24t worth of bubonic plague instead.

You get a pali or a straight. Once you unleash it on an enemy (connected to your land) they will have to use 8 tiles in order to kill it. A sort of distraction.
