Since Musharaaf left power in 2008 the USA has controlled their nuclear triggers. That's why you don't hear any true nuclear sabre rattling between Indian and Pakistan anymore, just conventional warfare sabre rattling.

Prez Bill Clinton allowed them to get nukes because he didn't really care and wanted to play both sides of the India/Pakistan conflict. Even if Pakistan used every nuke, it wouldn't be enough to wipe out even India. Even their nukes are poo tier.
