This actually shows that it wasn't a highly-choreographed photo-op. He was just helping to unload some random aid materials, which include toys to keep small children occupied. They didn't go out of their way to show him carrying something important like medical supplies.

1) Because it's a gesture. Yes, it's being done largely for the sake of appearances. That doesn't mean it's deceptive. The point is to show his values.

2) To see the damage and aid efforts with his own eyes. People lie to their superiors. A good leader must inspect a sampling of things personally. Again, this is a demonstration of his values, and of the way he intends to behave as president.

3) To set an example. A good leader should show that all necessary work is honorable by occasionally getting his hands dirty doing the simplest menial tasks. If the president can lug boxes or sweep a floor, nobody should feel ashamed to do such work.
