There's a good chance it ends exactly the same way, only with more Starks alive and Dorne not pissed at Robert/Lannisters.

I would say it's about 60/40 of it going down the same way. You forget that many houses are blinded by the oaths sworn by their ancestors to the conquering egg, or at the very least to the throne.

It's not clear how much Lyanna told Ned before she died, but at the absolute minimum he knows that Rhaegar is the father of Jon.

Everyone knew the Mad King was insane and most lords didn't like him. There are also some hints in the book that Ned's dad might have been involved in some kind of conspiracy to secede or overthrow him (some of his actions, like fostering and bethrothing kids with non-northern families are strange). So maybe he was looking for an excuse to go to war, or Aerys was aware and would have killed him anyway. Robert was also impulsive and violent.

So it kind of sucks that she didn't tell anyone, but it could have been a case of her and Rhaegar realizing that telling people wouldn't have helped anything. Either way, the Martells, Baratheons, and Starks would have been pissed because the two of them broke agreements. And as soon as Aerys killed Rickard a war would have happened anyway. It's also probably important to remember that Rhaegar was single-minded and only really cared about the prophecy, and he probably got Lyanna to think the same way for a marriage to happen; maybe they were so focused on producing a new Aegon that they didn't care about a war happening, or were thinking like cultists that cut off family ties.
