Because it was? Hospitals were for profit. Otherwise they wouldn't have existed. Care done for charity was either a goodwill effort by wealthy hospitals or "charity" work done by churches and their hospitals. I remind you religious hospitals weren't really charity either since they were paid for by the parishioners.

Actually as far as unfunded liabilities go you have a bigger threat from social security and medicare, which make up the bulk of your budget. That's one major reason people hesitate to add yet more """"free"""" care from the state. Nobody's willing to pay for it, except those on the bottom who expect the bill will be picked up by someone else anyway.

What's really fucked the US healthcare system isn't the idea of private insurance or private care (both still exist in countries with socialized healthcare, and for good reason), but rather an unfortunate compounding problem of how the legal system works (everyone gets to sue and payouts can cripple an entire hospital unless they too are insured to the eyeballs). This creates immense legal costs other countries just don't have. Insurers capitalize on this and inter-state restrictions making people drop in and out of care moving between states and jobs. It's all a legal nightmare and each complication is another layer of bureaucracy and juicing of fees.

But you've also said hospitals can't refuse people care. So you have a shit ton of the poor and uninsured and illegal who get care to the tune of millions from the hospital. So who pays that? You do! Of course. It's the most inefficient form of socialized medicine and yet again the risk and cost of these patients are compounded into 10000 dollar bandaids for the paying.
