It's from a time when England had like 10 different names for their entire population. Having so many with the same name necessitated an arseload of nicknames, but there was only so much you could do with calling one Will, on Willy, one Tall Will, one Our William etc, so finally they just did some rhyming-slang style nicknaming and changed Will to Bill and Rick to Dick, just to squeeze in another kid with the name that wasn't deformed enough to be called One-eyed Willy.
