The real answer is that this line is not actually in the books. Tolkien was an expert world builder, and he would've been well aware of that line being out of character for an orc. It was added by one the writers of the movie, who is not as good a writer as Tolkien.

You don't think that a warrior race who prefers axes in battle have some fundamental understanding of what happens when you plant one in someone's brain? No concept of basic anatomy by virtue of them battling orcs for millenia? He doesn't claim to be a neurologist, but a rudimentary understanding of "I move my axe his body twitches" is fairly straightforward.

The whole point of Tolkein's universe is that the world lost great knowledge and wisdom through the ages as the other races (primarily Elves) faded from Middle-Earth. Dwarfs were ancient Jews who were obsessed with engineering and were the technological dynamos of their time -- it wouldn't be unreasonable for them to understanding the anatomy of a human body.
